Remote consultation, done responsibly

Telemedicine in India operates within the Telemedicine Practice Guidelines issued in 2020, which set expectations around who can consult, patient identification, consent, record keeping and which medicines can be prescribed remotely. A good platform makes those steps part of the flow rather than extra paperwork.

It also has to work in practice: video that falls back to audio on a weak connection, chat for follow-ups, reports shared before the call and prescriptions a pharmacy can trust. We build telemedicine products with those details engineered in.

Challenges

Telemedicine realities

  1. Unreliable connections

Calls drop or freeze for patients on patchy mobile data.

  2. Doctor verification

Patients and platforms need to confirm a doctor's registration before consulting.

  3. Prescription limits

What can be prescribed depends on the consultation type and patient history.

  4. Doctor availability

Matching on-demand requests to scheduled doctor time is hard.

Telemedicine features we build

  • Adaptive video and audio

WebRTC calls through an SDK such as Agora or a self-hosted stack such as LiveKit, with audio fallback.

  • Doctor onboarding checks

Registration details captured and verified before doctors go live.

  • Guideline-aware e-prescriptions

Templates carrying the doctor's details, registration number and digital signature.

  • Asynchronous chat consults

Text, images and reports shared for follow-ups.

  • Queues and scheduling

Instant consult queues and booked slots with doctor availability rules.

How it works

How a teleconsultation runs within the guidelines

Identity, consent and prescription rules are steps in the flow, not paperwork afterwards.

  1. Patient app

Step 1: Patient requests a consultation

From the instant queue or a booked slot, with symptoms, photos and reports shared before the call.

  2. Platform

Step 2: Identity and consent recorded

The patient is identified and consents to a remote consultation, as the guidelines expect.

  3. Doctor

    Step 3: Verified doctor joins

Only doctors whose registration was checked during onboarding can take consultations.

  4. Video service

Step 4: Video, with audio fallback

The call adapts to bandwidth, drops to audio when needed and reconnects both people to the same session.

  5. Doctor

Step 5: E-prescription within the rules

Templates carry the doctor's registration and signature, and limit medicines by consultation type.

  6. Patient

Step 6: Pharmacy and chat follow-up

The patient sends the prescription to a pharmacy of their choice and follows up by chat with reports.

Plain-English glossary

Telemedicine terms, in plain English

WebRTC
The open technology browsers and apps use for real-time video and audio. It adjusts quality to the connection, which is why a call can drop to audio rather than freeze.
Video SDK
A ready-made kit from a video provider that handles calls, usually charged by minutes used. The alternative is hosting your own WebRTC servers, which moves cost to infrastructure and upkeep.
Registered medical practitioner
A doctor registered with the National Medical Commission or a state medical council. Only registered practitioners may consult remotely, so the platform checks registration before a doctor goes live.
Asynchronous chat consult
A consultation by chat, where patient and doctor reply at different times with text, photos and reports. It suits follow-ups that don't need a live call.

Patient app. When the patient's connection weakens, the call switches to audio instead of freezing, and reports shared in chat stay available throughout.
  • Telemedicine software doctor console instant queue in a web browser, with 3 entries
Doctor console. Doctors take patients from an instant queue or booked slots, with symptoms, photos and reports ready before the call starts.
  • Telemedicine software doctor console e-prescription in a web browser, with input fields and an action button
Doctor console. Prescription templates carry the doctor's details and registration number, and only allow medicines permitted for that type of remote consultation.

Cost drivers

Telemedicine cost factors

  1. Video infrastructure

Third-party video SDKs charge by usage; self-hosted WebRTC shifts cost to servers and maintenance.

  2. Consultation models

Instant, scheduled and chat consults each add distinct flows.

  3. Pharmacy and lab links

Connecting prescriptions and test orders to fulfilment partners adds integration work.

FAQ

Frequently asked questions

What do the Telemedicine Practice Guidelines mean for our app?

In practical terms, the app should confirm that consulting doctors are registered medical practitioners, identify patients appropriately, record consent, keep consultation records and restrict prescriptions according to the guideline categories for remote consults. We build these as product features: onboarding checks, consent steps, structured records and prescription templates. Your medical and legal advisers should confirm the final interpretation for your service.

How do video consults work on poor networks?

We use WebRTC-based video that adapts resolution to available bandwidth, and let either side switch to audio-only when video struggles. Reports and images can be shared in chat before or during the call, so they don't depend on the video stream. If a call drops, reconnecting returns both people to the same session. We test on low-end Android phones and throttled networks before release.

Can the platform connect to pharmacies for medicine delivery?

Yes. Once a doctor issues a prescription, the patient can send it to a partner pharmacy through an integration or order through your own pharmacy module. The pharmacist reviews the prescription before dispensing, as with a paper one. We keep the patient's choice of pharmacy explicit rather than defaulting to one, which matters for trust and for compliance considerations.
